Fixed term temporary full time, 27.05.2024 up to 11.05.2025

This role will work in partnership with other providers of primary health services for clients with chronic health disease conditions, both within the North West HHS and with external providers.

Assist clients by acting in a liaison and support role to ensure that they understand and are able to actively participate in their own health care.

Provide direction and information to Aboriginal and Torres Strait Islander people who live with, are at risk of, or have family members with chronic diseases. Provide this same cultural support and liaison to staff working with these clients and their families.

  • base salary ranges between $2686.90 - $3694.90 p.f. (HWF3).

Role requirements

  • Identified role: to be filled by a member of the Aboriginal and/or Torres Strait Islander community.
  • This position requires the incumbent to have successfully completed the Australian Qualifications Framework (AQF) Certificate IV in Aboriginal and Torres Strait Islander Primary Health Care (PHC) or equivalent.
  • Potential applicants are advised that the Commission for Children and Young People and Child Guardian Act 2000 requires Queensland Health to seek a ‘working with children check; from the Commission for Children and Young People and Child Guardian prior to appointment to this position.
  • This position may require the incumbent to operate a government vehicle and an appropriate licence endorsement to operate this type of vehicle is required (Queensland ‘C' class licence). Proof of this endorsement must be provided before commencement of duty.
  • Pre-employment screening will be conducted for the successful applicant.
  • VPD risk role: Hepatitis A/B, Measles, Mumps, Rubella, Varicella, Pertussis, Tuberculosis.
  • A 3-month period of probation may apply.
